Jackson Ridge and Maquoketa Care Centers are seeking a compassionate and dedicated Licensed Social Worker to join our care team. This role supports the emotional and psychosocial well-being of our residents by providing individual counseling , facilitating group therapy sessions , and offering family counseling services .
Responsibilities:
- Conduct one-on-one therapeutic sessions with residents.
- Collaborate with interdisciplinary staff to support residents’ overall care plans.
- Provide counseling and support to families navigating long-term care decisions.
- Develop and lead multiple group therapy programs tailored to residents’ needs.
- Collaborate with our Nursing Team and Physician to maintain best outcomes.
- Proficiently chart and document sessions and services provided.
Requirements/Qualifications:
- Active Iowa licensure as a Social Worker (LSW or higher).
- Experience working in nursing homes or long-term care.
- Experience working with patients with dementia induced behaviors.
