Job Description

The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco.  The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve.

We are seeking a Pediatric Nurse Practitioner for a Full-time, Part-time, As-Needed, or On-Call role. Spanish proficiency is strongly preferred.

Responsibilities

• As a member of a health care team, the PNP   Assumes direct responsibility for the health care of children and adolescents, including well baby visits, physical exams, and frequently occurring acute illnesses and chronic diseases of children.
• Must comply with standardized procedure agreements written by department head in Pediatrics.  Participates in the development of these procedures.  
• Refers to physician when appropriate.
• Maintains continuity of care for patients by making referrals to community agencies, and providing follow-up for hospitalized and referred patients.
• Collaborates and consults with physicians and other health care workers on a routine basis in regard to case load and before initiating medical care not covered by protocols.
• Incorporates into plan of care provisions for preventive measures to improve patient's health care status.
• Participates in health center provider, departmental CME and quality assurance meetings as determined by Associate Medical Director.
• Other duties as assigned.
• Provision of services may be at the Excelsior Pediatric Clinic or at Shotwell Pediatrics.

Qualifications

• Board Certified Pediatric Nurse Practitioner (PNP)
• Fluent in Spanish preferred.
• Passion for community health and serving diverse populations.

Compensation

• Pediatric Nurse Practitioner (PNP): $133,008 – $146,370

As a condition of employment, all candidates are required to meet specific immunization standards. Documentation of current immunizations, including Varicella, TDAP, COVID-19, MMR, and Hepatitis B, as well as TB test results valid within the last two (2) years must be submitted before the start date. Failure to provide this documentation may result in a delayed start or withdrawal of the job offer.
