RN Case Manager
Location: Brooklyn, NY

Details:

• Schedule: Day Shift | Monday–Friday, 8:00 AM–4:00 PM (Every other Saturday required)
• Contract: 13 Weeks
• Weekly Pay: Local – $2,545/week ($67.87/hr) | Travel – $2,896/week ($77.23/hr)

Role Overview:
The RN Case Manager coordinates and ensures delivery of patient care, supports discharge planning, conducts utilization review, and collaborates closely with physicians, social workers, and interdisciplinary teams to facilitate timely and effective patient outcomes.

Requirements:

• Active NY State RN License
• BSN required
• Minimum 3 years of direct patient care experience
• BLS and ACLS certifications
• Case Management experience preferred
• Strong communication and care coordination skills
