**Responsibilities**
+ Coordinates the provision of hospice services for each patient and family
+ Direct and follow established policies and procedures as related to the scope of practice
+ Maintain continuity of patient care in a hospice setting
+ Proactive, collaborative team member in the hospice environment

- Current, active, and unrestricted Registered Nurse (RN) licensure in state of practice
